- Log into your account. If you are a registered patient, your username is your email address and password, which you used during the initial setup
- If you do not have an account, fill out the signup section to create your online account. If you need additional assistance feel free to reference our enrollment guide
- Once you are logged in or enrolled, please select the manage appointments option in the top right corner, the system will show your upcoming appointments. You will see a patient forms button on that page that takes you to the forms for your appointment
