Here are the steps to Enroll to complete your forms online:

1. Click on enroll on the top right corner
2. Enter your First Name, Last Name, Date of Birth, and Mobile Number
3. The system will send you a Six Digit Activation Code, Enter it, and hit submit
4. Now you will see the complete enrollment form, fill out all the required information
   
   

This is a one time process. In the future, you can use this enrollment to just have to log in, fill out forms, and schedule appointments online.
