Please follow these steps to reset your password:

1. Go to the patient scheduling portal HERE
2. Click on login on the top right corner
3. Select 'Forgot Password' down below
4. Enter the details and submit

Once you submit, you will receive an email with the link to reset the password which will be valid for 30 minutes. 

As a Reminder

The scheduling site and your patient portal have different logins. If you have scheduled online before, your username will be your email and the password will be the one you created at the. If you have not scheduled before, you can easily enroll with us. You can begin the scheduling process by choosing your exam and location of your appointment. After that, there will be an enrollment screen where you can quickly create a username and password to use whenever you would like to schedule or manage your appointments online.
