On the patient scheduling portal here 

1. Click on enroll on the top right corner.
2. Enter your First Name, Last Name, Date of Birth, and Mobile Number.
3. The system will send you a Six Digit Activation Code, Enter it and hit submit.
4. Now you will see the complete enrollment form, fill out all the required information.

This is a one time process. In the future, you can use this enrollment information to log in and book your appointments online.
