On our scheduling portal:

1. Click on Enroll on the top right corner
2. Enter your First Name, Last Name, Date of Birth, and Mobile Number
3. The system will send you a Six Digit Activation Code, enter it and hit Submit
4. Now you will see the complete enrollment form, fill out all the required information and click Submit

This is a one time process. In the future, you may use this enrollment information to log in and book your appointments online.
