On our patient scheduling portal:

1. Click on enroll on the top right corner
2. Enter your First Name, Last Name, Date of Birth, and Mobile Number
3. The system will send you a Six Digit Activation Code, Enter it and hit Submit
4. Now you will see the complete enrollment form, fill out all the required information and click Submit
   
   

It's a one time process, in the future, you just have to log in and book your appointments online.
