Please follow these steps to reset your password:

1. Go to our patient portal HERE
2. Click on Login on the top right corner
3. Select 'Forgot Password' down below
4. Enter the details and submit

Once you submit, you will receive an email with the link to reset the password which will be valid for 30 minutes. 

As a Reminder

The scheduling site and your patient portal have different logins. If you have scheduled online before, your username will be your email and the password will be the one you created when you enrolled. If you have not scheduled before, you can easily enroll with us.
