Your username should be your email address. It would be the email that you used to enroll into our system. Make sure you type in the entire email address, with the .com address included.

As a Reminder

The scheduling site and your patient portal have different logins. If you have scheduled online before, your username will be your email and the password will be the one you created when you enrolled. If you have not scheduled before, you can easily enroll with us. You can begin the scheduling process by choosing your exam and location of your appointment. After that, there will be an enrollment screen where you can quickly create a username and password to use whenever you would like to schedule or manage your appointments online.
