On our patient scheduling portal here:

1. Click on enroll on the top right corner.
2. Enter your First Name, Last Name, Date of Birth, and Mobile Number.
3. The system will send you a Six Digit Activation Code, Enter it and hit Submit.
4. Now you will see the complete enrollment form, fill out all of the required information and click Submit.

This is a one time process. In future, you can use this enrollment information to log in and book your appointments online.
