Your username should be your email address. It would be the email that you used to enroll into our system. Make sure you type in the entire email address, with the .com address included.

As a Reminder

The appointment dashboard site and your patient portal have different logins. If you have filled out forms or have checked in online before, your username will be your email and the password will be the one you created when you enrolled.
