- Choose the type of exam you would like to schedule from the dropdown.
- Choose your insurance plan from the dropdown and click "Continue"
- You will be prompted to answer a few questions to ensure we are scheduling the correct exam for you. At the end of the questions page, click "Continue"
- The next page will show you our current availability at each of our locations. Choose a time slot and location that work for you.
- You will be prompted to log in if you already have an account, or enroll to complete the process.
- On the final page, enter your referring physician's name, upload any documents related to the appointment and click "Schedule".
