On our appointment dashboard portal here:

1. Click on enroll on the top right corner.
2. Enter your First Name, Last Name, Date of Birth, and Mobile Number
3. The system will send you a Six Digit Activation Code, Enter it and hit submit.
4. Now you will see the complete enrollment form, fill out all the required information.
5. Keep your cursor on the form and press Tab key or Scroll down to hit the submit button. It's at the bottom.

It's a one time process, in future, you just have to log in to access your appointment details here.
