Here are steps to Enroll for an appointment dashboard account.

On the login/enrollment page.

1. Enter your First Name, Last Name, Date of Birth, and Mobile Number
2. The system will send you Six Digits Activation Code, Enter it and hit Submit.
3. Now you will see the complete enrollment form, fill out all the required information.
4. Keep your cursor on the form and press Tab key or Scroll down to hit Submit button. It's at the bottom.

This is a one time process. In the future, you can use this account to log in and access appointment details
