To schedule an appointment:

1. Log into your account (if you do not have an account, you will need to enroll with the exact patient information that was provided at the time of service. i.e. Daniel Smith vs. Dan Smith)

2. You will be navigated to the orders page. Click "schedule" to start scheduling your order

3. Complete the pre-appointment questionnaire

4. Scroll down to the location you would like your appointment, and select a time

5. Complete your insurance information

6. Click "Schedule" to schedule your appointment
