• Eligibility: Reserved for patients 18 to 64 years old
• Includes: A physical touch exam or a well-woman exam
• Scheduling: Can be scheduled exactly 365 days from the previous adult annual
• Important Notes:
  • Cannot replace or be combined with a regular follow-up or sick visit
  • Must be scheduled separately from other medical concerns
