Southwest Care Center – Transfer of Care (TOC) Update

TOCs Available @ Harkle & Galisteo Clinics

Harkle Clinic – Providers Accepting TOCs:

  • Tim Gannon, PA
  • Andrew Pacheco, NP

Galisteo Clinic – Providers Accepting TOCs:

  • Alec Chan, PA

Instructions:

  • Pediatric patients do not require a TOC. Please schedule pediatric patients with a provider currently accepting new patients (Sarah Mele or Jennifer Trinh at this time).
  • Patients aging out of pediatrics also do not require a TOC, however require a 40min appointment to establish care with a PCP accepting new patients.
  • You only need Clinic Manager approval if the patient’s current provider is still actively working at SCC.
  • If the patient’s original PCP is still at SCC, submit a patient case to the Clinic Manager. Both the provider and Clinic Manager must approve before scheduling.

  • Appointment notes for these scenarios should read:

    “TOC from [previous provider name] — per requester; see Doc ID [insert Doc ID where Clinic Manager approved TOC]”

  • Check the provider’s scheduling grid for specific TOC guidelines.

  • Ensure that when completing a TOC, you update the patient’s provider in QuickView and click save.

TOCs Not Available @ Jefferson Clinic

Jefferson clinic does not have providers accepting TOCs at this time.

Patient Communication Script:

*"We appreciate and understand your request. At this time, SCC does not have any providers accepting new patients or Transfers of Care at the Jefferson clinic, as their panels are currently full. However, we can add you to our waitlist for a new provider when one becomes available. While we do not have an estimated timeline, we are actively working to recruit providers to meet the needs of our community.

In the meantime, we recommend that you continue care with your current provider. If you’d like, we can schedule an appointment with them to address your health needs. Please let us know how you’d like to proceed."*

Waitlist Instructions:

  • Ensure the waitlist includes patient preferences, such as provider type and preferred clinic location.
  • Follow the approval process if the patient’s original PCP is still at SCC.
